What about the company name, main contact, and address? I assume it's PCASDR, but what about the other info?

This is a general form that they use for everybody entering the event. You can either write "N/A" or leave stuff blank that does not apply. They won't kick you off for too much info. They won't be checking if you paid your taxes at the gate. Now if you have been donating money to al qaeda, it could be a problem
